不同术式治疗膝关节内侧间室骨关节炎患者对术后疼痛程度及膝关节功能的影响

目的 比较单髁置换术(UKA)与胫骨高位截骨术(HTO)治疗膝关节内侧间室骨关节炎(OA)患者术后疼痛程度与关节功能恢复情况的差异。方法 回顾性分析2023年3月至2025年3月南阳市中医院收治的198例膝关节内侧间室OA患者的临床资料,按照不同手术治疗方式分为UKA组(n=102)和HTO组(n=96)。随访6个月,记录并比较两组手术时间、总失血量、术后引流量、视觉模拟量表(VAS)评分、美国特种外科医院(HSS)膝关节评分、膝关节活动度(ROM)及术后并发症发生率。结果 UKA组患者手术时间和总失血量显著低于HTO组,术后1、3、5 d UKA组VAS评分显著低于HTO组,术后6个月UKA组HSS膝关节评分和膝关节ROM显著高于HTO组,并发症发生率低于HTO组,两组比较,差异有统计学意义(P<0.05)。结论 在适应证范围内,相较于HTO,采用UKA治疗膝关节内侧间室OA患者,能够缩短手术时间、减少术中出血,更有效地减轻术后早期疼痛,促进膝关节功能和活动度恢复,并发症更少。

Abstract:

Objective To compare the differences in postoperative pain intensity and joint functional recovery between unicompartmental knee arthroplasty(UKA) and high tibial osteotomy(HTO) in patients with medial compartment knee osteoarthritis(OA). Methods A retrospective analysis was conducted on the clinical data of 198 patients with medial compartment OA of the knee admitted to Nanyang Hospital of Traditional Chinese Medicine from March 2023 to March 2025. The patients were divided into the UKA group(n = 102) and the HTO group(n = 96) according to different surgical procedures. All patients were followed up for 6 months. The operation time, total blood loss, postoperative drainage volume, visual analogue scale(VAS) score, American Hospital for Special Surgery(HSS) knee score, range of motion(ROM) of the knee, and incidence of postoperative complications were recorded and compared between the two groups. Results The operation time and total blood loss in the UKA group were significantly lower than those in the HTO group. The VAS scores on postoperative days 1, 3, and 5 were significantly lower in the UKA group compared to the HTO group(P <0.05). At 6 months post-surgery, the HSS knee score and knee ROM in the UKA group were significantly higher than those in the HTO group(P <0.05). The incidence of complications in the UKA group was significantly lower than that in the HTO group(P <0.05). Conclusion Within the scope of indications, UKA for patients with medial compartment OA of the knee can shorten operation time, reduce intraoperative blood loss, more effectively relieve early postoperative pain, promote the recovery of knee function and ROM, and result in fewer complications.
